self-directed support - helping people live the lives they want
Self Directed Support
New Croydon Independent Support Brokerage Service
This is a new partnership arrangement between DisabilityCroydon, Age Concern Croydon, Advocacy Partners and Mind in Croydon.
The service is has been set up to provide support brokerage to people with social care needs in the London Borough of Croydon.
What is Self Directed Support?
Self Directed Support is a new way for you to get the support you need to get on with your life. It gives people more choice over the way they live their lives.
What can a Support Broker offer ?
Support brokerage offers people choice over who they wish to support them through self directed support. A person can choose to work with their support broker as well as others such as family and friends, support workers and their care managers.
A Support Broker can help you to create your plan, detailing how you will spend your money to meet your needs agreed with your care manager. They can also support you to seek out and try new opportunities, organise services and create a budget in order to get their own plan agreed by the local authority.
A Personal Budget is a sum of money that the Local Authority will give to cover the costs of your support needs. They will calculate your personal budget following an assessment where they will discuss the care and support that you require. You may need to contribute financially to your personal budget. This will depend on the result of a financial assessment.
If you would like more information about Self Directed Support, Support Brokerage or a Personal Budget, contact Social Services on 020 8726 6500 for a Community Care Assessment and ask to be referred to Self Directed Support. If you would like to have an informal chat with the Independent Support Brokerage Service (CISBS) first to discuss the process, please call;
